Outpatient Physical Therapy

 

Based on your visit to the ED (Emergency Department) today, it has been suggested that you have an evaluation by an Outpatient Physical Therapist.

Your choice of physical therapy provider is up to you.

If you would like to have physical therapy (PT) at a UCHealth Clinic, you have many choices in Northern Colorado which includes:

  • Longmont

  • Boulder

  • Loveland

 

A referral to PT has been placed within our computer system. You should expect a phone call within the next few days to set up those visits. You will be able to schedule an evaluation visit and a follow-up visit with one of our many specialized Physical Therapists. If you would prefer not to wait for that phone call, please call 720.718.5400 to schedule. 

If you would prefer to get PT services outside the UCHealth system, you may use the paper referral form given to you today at the clinic of your choice.

Insurance

If you have Medicare or Medicaid, you will need to talk with your Primary Care Provider (PCP) about your referral to PT. Your insurance provider requires that your care be overseen by a doctor, so your PCP will need to approve PT services. If you do not have a PCP, please use this link to find a doctor: https://www.uchealth.org/provider/.

If you have a commercial insurance plan, we suggest you call the number on the back of your insurance card to check your PT benefit.
